Church Mutual Policy Holders

To Report a Claim by Telephone
To report a claim or to check the status of a claim by phone, call (800) 554-2642, then select Option "2" when prompted.

Non-emergency claims may be reported during regular business hours, which are 7:00 a.m. until 5:45 p.m. Central time, Monday through Friday.

Emergency claims may be reported at anytime. Church Mutual claim staff remain on-call round-the-clock and may be reached through our emergency answering service.

Before you call, it is helpful if you have your account and policy numbers, a description of the claim or accident phone numbers where you may be reached, and any other facts regarding the claim that may help our claims representative process your claim promptly.

    Claims reporting forms are included with every Church Mutual policy. You can fax these forms--or forms you have downloaded from this Web site--to Church Mutual's Claims Department at (715) 539-4651.

    Submitting a claim has never been easier. Use the appropriate Online Reporting form to provide all pertinent information for your claim, and click submit.

    To Report a Claim Using E-mail
    You can use e-mail to notify us of a claim and to correspond during the settlement process. Initial e-mail claims reports should be sent to claims@churchmutual.com. Upon receipt, we will direct your e-mail to the appropriate claims representative.

    If you notify us via e-mail, please provide a description of the damage or injury, including how and when it occurred. For injuries, provide the name, address and phone number of the injured person(s). Please also provide your name, policy and account numbers and the name and address of the Named Insured on your policy. So that we can respond to you in a timely fashion, include your phone number and the best time to reach you.

    Our claims reporting forms are available in PDF format for download directly through our site.

    To use these forms you will need Adobe Acrobat Reader.

    To locate and use blank claims reporting forms:

    • Click "Claims Reporting Forms" here or from the sub-menu to the left
    • Choose the appropriate form under "Download Forms For Printing". This will open a PDF form.
    • Print the form.
    • Complete the form by hand or with a typewriter.
    • Mail or fax the form (see above for addresses and phone numbers).

    Please contact Church & Casualty @ (800)995-7525 between 8:00am - 4:30pm M- Th and 8:00am - 4:00pm on Fridays.

    Reference your organization's name/city and ask to speak with your Customer Service Representative. They will direct you in the necessary steps to file your claim.
